Elevate Health Australia (Elevate) will ensure that your confidentiality is protected as we abide by The Australian Privacy Principles which set minimum standards covering the legitimate use of personal information. Elevate collects personal information when necessary, when given voluntarily and which is stored safely. Our data storage systems ensure that your information is stored safely and is protected and will not be accessed without authority and shared with any third party without your consent or disclosure is required by law. You have the right to access your records.

Scope
Elevate respects privacy and is committed to protecting personal information. This Policy outlines our commitment to meet the obligations which we hold in regard to managing personal information we collect regarding our clients, potential clients, contractors and others. Personal information relates to information that can be used to identify that individual.

Elevate is bound by the National Privacy Principles contained in the Privacy Act 1988.

Elevate collects personal information in order to conduct our business, to provide our services and to meet our legal obligations.

Elevate provides a plan management services, including providing information and advice to clients and parties related to these matters, statutory authorities, and a wide range of service providers.

Personal information can include gender, age, claim or case number, home address or phone numbers, email and language. Health information can include diagnosis, clinical opinions and functional assessments.

In the operation of our business, we are frequently required to collect, use and/or disclose both personal and sensitive information. In most cases, if the personal information we request is not provided, we are unable to supply the relevant service.

Elevate will generally collect personal information by way of forms filled out by us and our clients, during face-to-face meetings, interviews and telephone conversations and from third parties.

Elevate uses and discloses personal information to support our clients with their individual plan management services.

  • Your advocate, family member or guardian
  • A service provider, a supplier of equipment
  • Other companies or individuals who you give permission and who are providing you services
  • Any other party that authority has been provided to disclose

 


How do we keep personal information accurate and up-to-date?

Elevate endeavours to ensure that the personal information we hold is accurate, complete and up-to-date. We encourage contact in order to update any personal information we hold.


You have the ability to gain access to your personal information
Subject to the exceptions set out in the Privacy Act, you may gain access to the personal information which Elevate holds. We will require you to verify your identity and to specify what information you require and we will advise you of the process.
