Dhruv is an experienced social worker with a strong foundation in supporting complex psychological illnesses. His expertise is grounded in trauma-informed practice, crisis intervention, and functional behavioural assessments. Dhruv has extensive experience navigating systems such as the NDIS, with a background in social work, support coordination, and behaviour support. David has been practising as a social worker for over 40 years in social and community welfare, supporting people who care for elderly relatives and family members with disabilities. For the past seven years, he has been working as a Specialist Support Coordinator. David enjoys helping NDIS participants make sense of their plans and make the best use of the funding provided.

With over 7 years of experience in aged care and disability, Jake is a dedicated and compassionate Occupational Therapist passionate about helping individuals lead more independent and fulfilling lives. As the founder of Peak Potential, Jake partners with Elevate Health to support our clients. He specialises in home modifications, equipment prescription, home and living assessments, assistive technology assessments, and ongoing therapy, including functional capacity assessments. Jake is driven to provide client-centred care that empowers people to achieve their goals and enhance their quality of life.

Vanessa has over 30 years’ experience across disability, mental health, forensic and child protection sectors. Vanessa has extensive leadership in NDIS clinical compliance and quality assurance. She has proven expertise in trauma-informed, person-centred, and evidence-based practice. Her former senior roles involved working across government (Corrective Services, Health, FACS) and leading NGOs. Vanessa has a high-level experience with high-risk, vulnerable and forensic populations. Vanessa was also involved with strong report writing for Guardianship Boards, MHRT, judiciary and regulatory panels.
